Where has "The Pillars of the Earth" by Ken Follett been banned?

I have heard that the book, "The Pillars of the Earth" has been banned in certain school systems, for its explicit sexual content, and violence. I know it has been restricted to 10th graders and above in Fairfax, VA, but I was wondering where else this book had been banned or restricted. Please help. The only other school where Pillars of the Earth has been censored, as far as I know, is in Cleburne, TX where is was removed from a summer reading list.
